Clean Claim

Definition
A submitted claim that contains all necessary data elements (correct member ID, date, codes, modifiers, signatures) to be processed and paid without additional information or investigation.

Why it Matters

Clean claims are paid quickly (often 14–30 days). "Dirty" claims are rejected or pended, delaying cash flow for months.

How it Works

The claim passes through an automated "scrubber" at the clearinghouse or payer. If it matches all logic rules, it is "Clean."
